Scammed or Phished: Who to Call

Have you been scammed or phished? Yes you have, if your identity has been stolen or your financial accounts been invaded without your permission. And now, who do you contact? First, if money has been stolen -- payments from the Indiana Department of Workforce Development (DWD) you will need to fill out State Form 57068 and a state police form that can be emailed to - uifraud@isp.in.gov  If you are employed, you will need to alert your employer and file forms with the state police, attorney general's office and/or the FBI.

If your information was stolen (identity theft), obtain the following forms:
-  Department of Workforce Development (DWD):  Form 57068
-  State police: Form 2464

- FBI:  http://www.ic3gov for an internet crime complaint.

Additional steps -

-  Take a screenshot of the attempted hack and send to - https://www.in.gov/dwd/contact-us/
-  If the link was opened that could have exposed your personal information - call the DWD Contact Center
-  And if your site has been altered in any way, change your password and ensure your payment options are being routed to your bank. If help is needed, call DWD @ 1-800-891-4499.
